Per Child: $100

For those enrolling multiple children: $ 85 per week or $80 for each child if enrolling for both weeks.

I understand that there may be elements of risk associated with activities on one of the New Life summer camps. I give permission for my child(ren) to participate in all activities at New Life and hereby release and agree to indemnify and hold harmless New Life from any and all claims of any nature arising from such participation.

In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the particular ministry director to hospitalize, secure proper treatment and to order injections, anesthesia, or surgery for my child(ren). I give permission for the nurses to administer “over- the -counter” medications to my child(ren) if necessary.

I hereby grant to New Life, the perpetual right to use photographs or video footage taken of my child(ren) as a result of participation in activities at New Life Camps.
